Thai Researchers Find Biological Clues to Long COVID Symptoms Lingering for Years

When the virus lingers: Thailand’s growing focus on Long COVID’s biological roots

Thousands of Thais who recovered from COVID-19 still suffer from debilitating symptoms years later — not because they didn’t rest enough, but because fragments of the virus may be silently triggering their immune system. New research from Thai institutions is shifting the narrative: Long COVID isn’t just fatigue. It’s a biological mystery driven by lingering viral debris.

SARS-CoV-2 spike proteins have been detected in the colon, bone marrow, and meninges up to four years after infection, according to studies conducted by Thai medical teams and published in peer-reviewed journals. These fragments, harmless on their own, appear to act like persistent alarm bells, keeping immune cells in a state of low-grade attack — even in people who had mild or asymptomatic cases.

This phenomenon, now widely discussed in Thailand’s medical circles under the working term “Long-Spike,” is gaining traction as a central theory explaining why symptoms endure. Evidence from the RECOVER project, adapted into Thai clinical analysis, shows that Long COVID patients have nearly double the levels of viral antigens in their blood compared to those who fully recovered.

Clinical patterns in Thai patients

Thai hospitals are now treating Long COVID not as a post-infection afterthought, but as a defined clinical condition. The Ministry of Public Health uses WHO-recommended screening tools to identify cases nationally, and the data paints a clear picture:

38.2% of patients surveyed at Thammasat University still had symptoms three months after infection.

22% of those patients continued suffering beyond 12 months — with some still symptomatic at the two-year mark.

Among those infected during the Omicron wave, over half reported lingering symptoms at three months, according to nationwide clinic surveys.

The most common complaints in Thai patients include crushing fatigue, shortness of breath, brain fog, chronic headaches, sleep disorders, and unexplained muscle pain. Women under 40, individuals with two or more chronic conditions like diabetes or hypertension, and those who required hospitalization during acute infection are at greatest risk.

Prof. Dr. Thiravat Hemachudha of Chulalongkorn University, a leading voice in the field, states: “We see patients who had one fever and went home — now, they can’t stand for more than ten minutes. The science says it’s not in their head. It’s in their tissues.”

Where Thailand stands on treatment — and what’s coming

Despite mounting evidence, Thailand has no approved therapy that directly targets persistent viral proteins. No drug exists to flush out spike fragments or quiet the chronic inflammatory response they trigger.

Instead, care is built around symptom relief. Nine public hospitals under the Bangkok Metropolitan Administration operate dedicated Long COVID clinics, offering pulmonary rehab, physical therapy, and psychological support. Private hospitals like Chularat 9 Airport Hospital and Samitivej have followed suit.

While international trials are testing drugs like nirmatrelvir, molnupiravir, and low-dose naltrexone to suppress residual viral activity, these remain off-label and unavailable through public health programs in Thailand. Research into monoclonal antibodies designed to neutralize spike proteins is also underway but has yet to reach Thai clinical trials.

In the absence of targeted drugs, traditional herbal remedies like Fah Talai Jone (Andrographis paniculata), turmeric, and ginger have become widely used by patients. Though no rigorous trials confirm their effect on viral persistence, anecdotal reports suggest they help ease inflammation and fatigue.

The silent companion: High blood pressure and stroke risks

Two other findings deepen the urgency of understanding Long COVID’s biological underpinnings.

A 35-year longitudinal study — the CARDIA data adapted for Thai demographics — shows that high blood pressure before age 40 accelerates brain aging, with visible white-matter damage appearing years earlier than in healthy individuals. This suggests that vascular stress from hypertension may combine with Long COVID’s neurological symptoms to amplify cognitive decline.

Separately, researchers have identified that during acute ischemic stroke, neutrophils release sticky DNA webs — called NETs — that physically trap clot-busting drugs. If similar immune traps form in small vessels of Long COVID patients, they could be blocking oxygen delivery to brain and muscle tissue, worsening fatigue and cognitive fog.

What patients should know today

If you’ve had COVID-19 and still feel unwell after three months, you’re not alone — and it’s not all in your imagination. Thai doctors now recognize Long COVID as a real, measurable medical condition with tangible biological causes.

The priority is simple: Get screened. Use the official questionnaire provided by the Ministry of Public Health. Seek care at a designated clinic. Focus on rebuilding stamina through structured rehab, not resting in isolation.

While drugs that eliminate viral debris are still years away, the body’s resilience can be supported. With proper therapy, many Thai patients report improved function within six months.

Awareness is the first step. Recognition is the second. And now — the science is catching up to the suffering.
